that guy hitting himself is part of a group called the shiah who were meant to help during a war against the Islamic prophets grandson hundreds of years ago. the shiah fled just before the battle and the grandson, along with many women and children died.

to this day the shiah regret doing that and believe that by hitting themselves every year during his birthday (or death anniversary, not sure) they will be forgiven for their cowardice
